Supporting and implementing community based digital work, which will include providing one to one and group support to patients.This will include working alongside other digital champion/connector roles within local communities and learning from peers within other local primary care networks Regular attendance at Digital Cafes within local communities, working closely with Health Connectors who will refer patients for support with getting online, using healthcare apps and accessing free devices Assist patients with access to their digital health record via the NHS App, whilst ensuring appropriate data protection measures are applied Record data on our clinical system, ensuring coding is in line with national and local requirements, and ensuring digital work can be measured and evaluated Working collaboratively with other members of team to develop new digital projects and ideas.
Develop links and partnership working with local and national partners such as businesses, councils, charities, and other individuals or groups Take part in community engagement and awareness raising events with a focus on current digital projects and opportunities Produce communications which promote and evaluate our digital enablement work in a format which is suitable for social media, newsletters and website content, and wider system sharing and learning Maintaining professional and personal development This post requires flexible working and scheduled working patterns to ensure service provision
