Our Medical Specialities are looking for a Discharge Facilitator to join it's friendly and supportive team to liaise with other members of the MDT and the patient's family and carers and with the support the nursing staff of the wards to ensure and facilitate speedy, efficient and timely discharges through a combination of the administrative and clinical elements of the role


You will work under the supervision, direction and guidance of qualified nursing staff and ward manager to initiate implement, assess and carry out discharge planning for patients from admission and coordinating this throughout their stay in hospital and will be the focal person within the ward environment to ensure that the patient and their families are informed throughout the discharge process to ensure a seamless journey and identify, in a timely manner, any onward service requirements.


You will be pivotal in liaising with families social services care providers and other members of the MDT to co-ordinate assessments prior to discharge.


Your role will be to plan and manage complete episodes of care, working in partnership with others, and delegating and referring as appropriate to optimize health outcomes and resource use, as well as providing direct support to patients and clients and appropriately define the boundaries of own practice, asking advice from nursing staff and senior sisters when unsure.


You will ensure that services are delivered effectively and efficiently against predetermined clinical performance targets over a 7 day period where appropriate requiring a proactive approach to promoting health and wellbeing, detection and prevention of vulnerability, preventing illness, reacting and responding to existing or potential ill health issues and contributing to reduction in length of stay or reducing unplanned hospital admissions.

You will be expected to conduct yourself and practice within an ethical framework based upon respect for the well
- being and safety of patients and clients for the clinical case management of a defined population and collaborate with a wide range of statutory and voluntary organisations to maximize the nursing contribution in health improvement, planning and delivery of care to individuals, families and communities.
