The post holder will work within the Transfer of Care Service with flexibility to cover as required.

The post holder will work as part of a service provided from 8.00 am to 8.00pm, 365 days per year, as negotiated with the Operational Manager.

Infection prevention and control is integral to this post. The post holder should at all times demonstrate effective infection control and hygiene practice.

To provide a confidential and comprehensive administrative service to the Transfer Of Care team.

To work as part of the overall Transfer of Care services administration team.


The Transfer of Care team provides expertise and support to A & E, medical assessment units, Frailty unit and base wards to facilitate complex discharges.


To use a range of work procedures and practice using theoretical knowledge to maintain effective filing and record keeping systems, including the indexing of files and retrieval of information using appropriate software programmes.

Contribute to the efficient and effective maintenance of office systems and procedures, carrying out administrative duties as required, including photocopying, faxes and mail-outs.

Provide and receive routine, complex or sensitive information and on occasion exchange information with patients, carers or relatives using appropriate tact.

May be required to exchange complicated information within the team.

Receive and screen telephone calls, resolving enquiries where possible, taking messages or re-directing calls as appropriate to facilitate timely and appropriate communications with others.

Have knowledge of procedures and ability to problem-solve as a central point of contact for staff. Take details of complaints / incidents, take details of staff absences. Work pattern can be unpredictable and work may be interrupted to deal with Queries.
