This is a great opportunity to join the Trafford living well team. The Post is for a MHP and is permanent and full time. The role is open to RMN's, Social Workers and AHP's.The post holder will undertake a key worker role, alongside an assessment and formulation role within the team.
The Living Well Service will enable timely access to mental health services when people need it most and provide support for people who might not traditionally meet the threshold or criteria of existing mental health services.
The team will be multidisciplinary and will also include third sector organizations. The areas that this team will cover are Trafford North, Central, South, West and Altrincham Health Alliance.This role will be to cover Central Trafford but there will be an expectation to move across other neighbourhoods for service needs.
You will need to have experience of triaging and signposting referrals as well as being able to liaise with multitude of agencies and professionals.
Trafford Living Well Service will be compassionately led, trauma informed, and recovery focused. CPD opportunities are available and personal interests/skills will be supported to develop and grow.
