Working arrangements
The role will be predominantly based in Chester city centre (CH1 1QE), with a requirement to work within tenants' homes and attend external meetings as required.
To develop, coordinate and drive a bespoke plan of support services for individuals and families working in partnership with local voluntary and statutory services, to secure the best outcomes for families in relation to welfare, safety, housing, education and health matters.

A proportion of our homes are therefore targeted at young people leaving care, families living in temporary accommodation, families fleeing domestic abuse, families with children on the edge of care.
This role is part of our front line multi-disciplinary support team working with children, young people and their families to coordinate and ensure the delivery of a range of a high-quality, responsive, and relevant services (internal and external) which address the needs of our tenants and remove barriers to their success.
In addition, we expect our support team to work directly with individuals and families to help develop their resilience and life skills and to encourage them to achieve their goals according to an agreed support plan.
Key responsibilities
With a professional background in social care, youth work, health, or other related area and an understanding of the challenges faced by children, young people and families, it is essential that you have previous experience of working with children and families/foster carers as well as a sound knowledge of safeguarding procedures.
- Build trusted relationships with families and young people prior to, during and after their tenancy within Grosvenor Hart Homes.
- Prior to moving in, work with each prospective tenant / household and the relevant statutory services to assess support needs and create a bespoke plan to meet those needs which moves them through to the next stage of their housing pathway and sets clear, measurable outcomes.
- Discuss, agree, and then implement a move in plan to smoothly transition each tenant into their home.
- Drive and oversee execution of each tenant's support plan
- removing barriers, helping to solve problems, keeping tenants motivated and focused on achieving the steps/ goals within their plan to its successful completion.
- Offering practical and emotional support to individuals and families as needed, including in times of emergency or crisis.
- Operate as the individual or household's named point of contact for GHH, responsible for keeping records and updating reports using an effective case management process.
- Work with GHH residents to facilitate neighbourhood projects to further social outcomes, e.g., community gardening, arts, cooking activities.
- Adhere to the safeguarding policies and procedures of GHH, taking an active role in the safeguarding of tenants.
- Participate in supervision and regular team meetings taking responsibility for own work and escalating concerns.
- Deputise for other members of the GHH team as required.
- Provide supervision to students or more junior members of the team as required.
- Provide feedback, progress reports and contribute to evaluations for internal and external stakeholders as required.
- Ensure there is a joined-up approach to supporting GHH tenants across a multi-disciplinary team.
In addition, the role requires the following responsibilities:

- To carry out duties, within the relevant legislation at all times and be familiar with the terms of all current legislation relevant to the role
- To ensure Health and Safety responsibilities are fulfilled, including:

- To ensure that all H&S responsibilities are fulfilled, and that safety and welfare is maintained.
- Plan enough time for work to be done in a healthy and safe way.
- To assume delegated authority to take aversive/proactive action where appropriate to prevent a potential situation escalating to an accident or health and safety failure.
- To complete any other duties as required from time to time.
