We are looking for a caring and motivated colleague to join our Adult Community Therapy Team in the Spalding area of Lincolnshire.
This team predominately supports patients who require rehabilitation following an illness, injury or operation.This can either be completed in a transitional care bed setting in a care home or within the patients' own homes.
Our aim is to support the patient to be as independent as possible within their own property.We are a enthusiastic and friendly team who work closely locally, within a sector and across the county. We have a strong supportive network and are keen to encourage our colleagues to develop within our Trust.
This position is to support patients who have been admitted into transitional care beds within a care home setting or visiting patients in their own homes to complete therapy care plans as part of their rehabilitation journey.
You will need to have an ability to travel around the local vicinity due to the community nature of this position.
You will also need to work with the team to monitor patients' care and support with other tasks such as completing onward referrals, ordering equipment and answering telephone enquiries.

- To work as a member of Adult Community Therapy delivering person centred care with particular focus on rehabilitation interventions, enabling individuals to achieve their optimum physical, psychological, social and emotional wellbeing
- To implement care as defined within individualised patient care plans, as directed by the senior staff, to patients in a variety of care settings within the community.
- To undertake assigned tasks supporting effective team working including elements of rehabilitation interventions and administrative tasks as delegated by senior staff
- To contribute to the service targets to prevent hospital admissions
- To maintain high quality standards in all aspects of care delivery, working in partnership with primary care colleagues, other health professionals, patients, carers, statutory, independent
- To support students as directed by senior staff
