£30,151 to £35,411 per annum, pro rata


Hours:25 hours per week


Location:
Bath and North East Somerset


Contract Type:
Fixed Term Contract, 31st March 2024.


Additional Information:

The Role
DHI are recruiting a Team Leader for the Social Prescribing Service in Bath and North East Somerset.

This service supports residents in B&NES to manage and improve their health, wellbeing and their resilience by connecting them into community services, groups,activities and other pathways.

The service plays a vital part within local surgeries to provide residents with holistic care.


You will be responsible for the supervision of staff working across five PCN's as well as ensuring that referrals for the service are well-managed and targets are met.


You will have the opportunity to work flexibly between home, DHI's head office in St James Parade, Bath and the Community Wellbeing Hub in Peasedown St John.

The role is ideal for someone who enjoys working with multiple stakeholders in a health and socialcare settings.
