You will need to be:


  • SWE registered
  • Eligible to work in the UK
  • The post is located within the Transitions Team years) which works with young people who have a disability, learning disability, autism, or who have or had an Education, Health, and Care Plan (EHCP), and has recently transferred to Communities, Adultsand Health Directorate.

The role involves:

  • Liaising with the 018 Year Service, education, and health to enable smooth transition of young people into adult services at the age of 18 years.
  • Providing sound direction and guidance to the team, ensuring professional standards and Best Practice act as key drivers within a culture of continuous service improvement, value for money and safer practice.
  • Ensuring compliance with the service's standards and processes and operate within the context of the service priorities and principles of increasing user choice and control over the services they receive.
  • Providing sound, clear, and responsive practice leadership and supervision, and promote the highest professional standards in social care
  • Working closely and collaboratively with partners to deliver an integrated, service that meets both current and future needs.
  • Working with the Transitions Team Leader supporting the day to day running of the team
  • Ensuring that the responsibility of safeguarding adults at risk is a fundamental element of this job and that the principles are embedded in all procedures, practices, professional advice, decisions and support.
