Description

Duties:


  • Work alongside and assist the Team
  • Oversee the daily supervision of other Care Coordinators in their team
  • To ensure that the office runs effectively and professionally
  • Long and shortterm rostering of care home staff ensuring appropriate cover is placed
  • Ensure that the care team has the right skills to provide care to each patient's care needs
  • Lead administrative processes for the Care Home Team
  • Ensure that Staff adhere to company and PCN policies and procedures
  • Ensure support to PCN Management Team when creating, and preparing Induction plans for each new member of the Care Home Team
  • Provide education and support to patients and their families
  • Monitor patient progress and adjust care plans as needed
  • Maintain accurate records of patient care

Requirements:


  • Good general standard of education
  • Management or supervision experience
  • Working with people who have complex health and care needs to improve their wellbeing
  • Experience of working in a dynamic and creative way, solving problems and prioritising work
  • Experience of working in a health and/or social care setting

Experience:

Care Coordinator: 3 years (preferred)

- supervisory: 1 year (preferred)


Licence/Certification:

  • Driving Licence (preferred)
  • NVQ Level 2 Health & Social Care (preferred)
