Positive Employment is currently recruiting for a Supported Pathway Officer based in Southwark.


The successful post holder will assist residents in securing their next stage housing, providing resettlement support so that they successfully move into and sustain their new home.

To provide appropriate floating support for 6 months and to ensure they are linked into necessary support services thereafter.


DUTIES AND RESPONSIBILITIES:


  • Assist and motivate residents to reach set goals and progress along an identified clear pathway for housing. Provide client centred support plans and intensive assertive keyworking support using reflective practice techniques.
  • Coordinate any necessary support services to assess residents ability to live independently and identify which services could assist them to sustain their wellbeing and tenancy and provide a time limited floating support service to ensure residents are linked into appropriate services after moving out.
  • Encourage residents to engage and participate in activities which address issues of social isolation and confidence building and to accompany them where necessary and appropriate.
  • Advocate for residents where required, providing practical support if needed to ensure that their rooms are habitable and safe and where necessary to accompany residents to appointments. e.g. Health Services, DWP, viewings.
  • Continually monitor and safeguard residents' physical and mental health and liaise closely with appropriate services.
  • Provision of training and workshops that enable residents to gain life skills.
  • Issue prescribed medication only in accordance to policy, on health authority instruction, and following training.
  • Ensure full occupancy and good management of bed spaces in the borough by meeting a fast void turnaround in accordance with the void procedure and targets and ensure that any necessary storage and disposal of resident's goods is done according to council procedures.
  • Encourage resident participation in hostel matters, investigating and responding to any suggestions and complaints and in running house meetings.

Personal Requirements:


  • Knowledge of the impact of Adverse Child Experiences and the principles of Trauma Informed Care when dealing with homeless people who have complex needs.
  • Knowledge of the Care Act and Housing legislation and the available support in relation to single homeless adults.
  • Knowledge of welfare benefits and other support that is available to residents of supported accommodation.
  • Health and Safety issues in relation to a supported housing setting.
  • Experience of working with homeless people with complex support needs.
  • Enhanced DBS Certificate.
  • 2 Years work references.

WORKING HOURS AND RATE:


  • Hours: 8am4pm Early shift/2am8pm Late shift 36hrs per week
  • Rate: £22.45 p/h Umbrella
