Are you looking for a Community Support Worker role with a special interest in Urgent Community Response? We are looking to enhance our ability to deliver Urgent Community Response across our Bristol and North Somerset localities.

This is an exciting opportunity to work as part of multi-disciplinary integrated network teams with a focus on delivering urgent care.

The main purpose of the role will be to assist clinical staff in the assessment and treatment of patients in order to prevent hospital admissions and facilitate early discharge.

You will follow prescribed aspects of care, supervising patients with personal and domestic living activities, adaptions of mobility aids and the provision and fitting of basic aids.

By joining us you will have many opportunities to undertake further training to develop extended skills and enhance patient care.

We deliver an 8am-8pm, 7 days a week service and working patterns are rostered to be able to provide this.
