CSS works to support patients who have had an acute stroke to leave hospital earlier by providing care and rehabilitation at home or in nursing/residential homes.
It is a citywide service made up of an inter-disciplinary team, consisting of Nurses, Physiotherapists, Occupational Therapists, Clinical Psychologists, Rehabilitation Assistants, Admin and Managers.
Your role will be to assist the multiple-disciplinary team in the day to day management and rehabilitation of acute stroke patients.
You will undertake assessments under the guidance of qualified staff, implement and progress rehabilitation programs, set and review Goals and at times provide Specialised enabling care support.
You will be responsible for the delivery of high standards of patient care and the efficient flow of patients through our service.
To carry a clinical caseload delegated by Occupational Therapists and Physiotherapists, plan and prioritise own work and report back to qualified therapy staff.
Provide treatment in hospital and/or in the community,
Measure activity and treatment outcomes
Conduct an initial assessment following protocol on patients delegated by Occupational Therapists and Physiotherapists.
Complete a cognitive assessment following protocol
Complete falls screening
Set up equipment for therapy and ensure it is safely used by patients, carers, staff and/or students
Teach, guide and supervise patients as individuals or in group
Please note this advertisement can be appointed as either a full time or part time opportunity.
The service is highly responsive to changing demands for care and you therefore need to be flexible in terms of working hours and where you are based.
The role will be based from SPARC at Beech Hill or other community bases as they develop and in time there may be the opportunity to work across the integrated stroke pathway including the Acute Stroke Unit at the Hallamshire hospital and SPARC at Beech Hill.
