We are seeking a Support Worker to join our friendly and dedicated team here at Liaise

Responsibilities:


  • To deliver support to the people in our homes with learning disabilities, mental illness and additional complex needs
  • To support our people in all aspects of daily living and community participation
  • To assist the people in our homes in leading a valued, fulfilled, healthy and active lifestyle, as independently as possible
  • To transport the people we support with learning disabilities and mental health between the service and daily activity settings to ensure they are safe and comfortable during the journey
  • To work in accordance with Liaise's policies and procedures, and all relevant legislation at all times
  • To work as part of a friendly and dedicated team, assisting with cleaning of the home and support our people to do so where appropriate. Including but not limited to, their meals and clothes, washing and drying
  • To assist the people we support to keep in contact and develop their network of support with family, friends and involved professionals

Requirements:


  • Effective team player and capable of working collaboratively with others
  • Strong communication skills
  • Takes pride in working with vulnerable people and takes safeguarding responsibilities seriously
  • Ability to operate our patient record system
  • Effective decision making and judgement
  • Strong planning and organisation skills
  • Preferably has successfully completed NVQ Level 2 in Adult Social Care (or equivalent)
  • Achieves all Health & Safety standards
