Description

We are seeking a full time Learning Disability & Mental Health Care Coordinator to join our growing multi-disciplinary team at Our Health Partnership, working within practices across the Kingstanding, Erdington & Nechelles Primary Care Network (KEN PCN).

As Care Coordinator you will be coordinating a process to support the needs of patients with Learning Disability or Mental Health Diagnosis.

You will need to build relationships with each practice within the PCN and work closely with other Care Co-ordinators within the PCN to ensure the needs of patients are met.

You will be responsible for providing support to practices to ensure that patients receive regular check ups, medication reviews and Care plans.

The care coordinator will ensure that the person's needs are met, will value the important of their choice and decision making in their life, and will actively seek to encourage his gifts and abilities.

Person Specification

Qualifications_

Essential

GSCE grade A
C (or equivalent) in Maths and English, or higher level qualification

Desirable

NVQ3 in Health & Social Care
Experience_

Essential

Experience in a patient/customer facing role

Desirable

Experience of working in primary care
Experience of working in a GP practice
Skills and Abilities_

Essential

Due to our location, access to a vehicle and ownership of a full, clean driving licence is essential
Excellent interpersonal and communication skills
Excellent empathy and listening skills
Prioritisation skills
Good IT skills

£23,104.00-£24,960.00 per year
