Job summary
Trinity Health Group PCN are looking for a Paramedic to join our growing ARRS team.

You will work in partnership with the practice's, as well as the wider PCN team in developing this role further.

Are you currently working or willing to work towards a relevant level 7 or equivalent academic qualification? Do you have experience of working with a multidisciplinary team to improve patient care? Is working in Primary Care something you have always wanted to do?


Within this role, you will provide clinical assessment for patients, support with acute and chronic disease management, promote health, measure and use data and information, develop and maintain excellent communication skills, work independently as well as being a vital member of the team, adhere to risk management requirements and ensure excellent quality of care is provided to patients.


Main duties of the job


To work as part of an MDT within the PCN to provide patient-focused care responsive to health need demonstrating strong communication at all levels across the organization.


To provide first point of contact direct access for patients of all ages with undifferentiated and undiagnosed problems seeking a primary health care service including on-call, telephone triage, face to face consultations, and home visits.

To assess, triage and manage patients within a variety of settings.

To order investigations, undertake and follow-up results and arrange appropriate management including seeking advice where appropriate.


Provide advice to patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service and other services where appropriate.

Actively take a personalised care approach and population-centred care approach to enable shared decision making with the presenting person.


Maintain accurate and contemporaneous health records within SystmOne appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient healthcare.


Communicate effectively with patients and their family and carers where appropriate communicating sensitive and complex information regarding their physical health needs, results, findings and treatment choices.

To proactively engage in daily debrief sessions to discuss patient care with a senior colleague.
