Are you a qualified exercise instructor/professional seeking new opportunities to support patients to exercise with a diagnosis of Heart disease?


We are seeking a dynamic individual with the enthusiasm to motivate and inspire our patients to engage in maintaining regular exercise as part of a healthy lifestyle.


You will work within the cardiac prevention and rehabilitation team with specialist nurses and an exercise specialist, with support from a wider multi-disciplinary team, to deliver a programme of exercise and education which adheres to the National Standards of the British Association of Cardiac Prevention and Rehabilitation and Core Components for Cardiac Rehabilitation.

This is an excellent opportunity to develop you knowledge, skills and experience in a specialist field.

As this is a hospital and a community based role, the ability to travel around Wandsworth is necessary.

The cardiac prevention and rehabilitation service provides specialist cardiac rehabilitation
input to both the Cardiology and Cardiothoracic patients including an outpatient service to the


local population


This includes:

1) In depth patient assessment, secondary prevention and monitoring of cardiovascular
risk factors including medication review and compliance with treatment
2) To provide a structured cardiac rehabilitation programme at a variety of venues
including SGH, Queen Mary Hospital Roehampton and local leisure centres providing
prescribed exercise, relaxation and stress management and on-going education.
There is also the option of home visits and individual exercise sessions as
appropriate.
3) Support and advice to both patients and their families.

4) Referral to relevant specialties and services to enhance recovery and cardiovascular
risk reduction.
5) To work with a wider multidisciplinary team to ensure provision of an individualised
cardiac prevention and rehabilitation service to cardiac patients with a variety of
conditions, within the outpatient or home setting.

6) To deliver cardiac prevention and rehabilitation exercise sessions in a variety of
venues and using remote access and digital tools.
7) To consistently meet the highest standards set by the British Association of Cardiac
Prevention and Rehabilitation (BACPR) and contribute to the National Audit of Cardiac
Rehabilitation (NACR)
