An exciting opportunity has arisen to work with an innovative and dedicated team supporting younger people with dementia across Northamptonshire.


The Community Support Worker role will work closely with the multi-disciplinary team (MDT), providing post diagnostic support and advice to both service users and their relatives.

The post will involve liaising closely with other agencies, including health, social care and the voluntary sector.

Advice and signposting regarding benefits and other entitlements

Working closely with MDT to promote service user's well being and living well with Dementia.

Group work with service users and relatives.

Making links with appropriate services and resources in the local community and signpost service users and relatives accordingly.

1:1 work with service users towards identified goals.

The team provides memory assessment and dementia diagnosis for adults under 65 years.

It's a small, county wide service, consisting of nurses, Occupational Therapist, Neuropsychologist, Assistant Psychologists and Administrative support.


The team is dedicated and passionate towards helping people to retain their independence for as long as possible and supporting them and their relatives from the point of diagnosis and throughout their journey.


NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community.

Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible.

This means many of our services can be provided at home, work or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
