Dementia Provision Coordinator- Who are we: Everton in the Community (EitC) is one of the UK's top sporting charities and firmly established on the world stage of community sports development.

About the opportunity:

As Dementia Provision Coordinator for EitC you will be responsible for coordinating the effective delivery of EitC's Dementia provision including the long-standing programme; Pass on the Memories, and support with the future plans for the Goodison Legacy.- The role will involve working closely with a range of stakeholders including community groups, Residential Homes, local authorities and NHS services to ensure clear referral pathways are established.


Who we are looking for:

We are looking for an individual who has at least 2 years' experience of working with and delivering activities for people with dementia as well as knowledge of behaviour change models/ strategies to promote better health and wellbeing outcomes.

Essentially, you will hold a Dementia specific qualification and have undertaken Mental Health awareness training.
If you are experienced in the co-ordination and delivery of evidenced-based support and engagement activity sessions for people living with Dementia and their carers and you have a genuine passion for helping support people to improve their health and wellbeing then we would love to hear from youNot only will you get to be a part of something special as we start a new chapter for Everton in the Community, but as part of this role you will support the on-going development of EitC's
