Salary:
£26,000 p.a. pro rata, plus contributory pension

Hours: 2 x posts: 22.5 or 30 hours per week

Contract:
One year fixed term contract, starting in January 2024

Great West Road Primary Care Network in partnership with Groundwork London and Hounslow Consortium are seeking to recruit two part-time Community Health and Wellbeing Workers (CHWW).

The CHWW is a new and rewarding opportunity that aims to make a difference to the quality of life for residents in Hounslow.

The CHWW will visit residents in their homes and community settings to discuss their health and wellbeing.

CHWW will provide support and advise on a range of health matters through community health education groups, signposting to other community services, identifying where individuals are eligible for services that have not been accessed, and reinforcing positive public health messaging.

As CHWW, weekly supervision will be provided by a lead GP, alongside support from a line manager. A full induction, including training will be provided in a range of health, clinical and community engagement topics.
