Description
A new and exciting role has become available within Knowsley Later Life and Memory service. This is part of a 12 month national pilot, working to improve diagnosis rates and dementia care within Knowsley.

Short listing 2nd March 2023

Interviews planned for 6th March 202- To work alongside the Dementia Diadem diagnosing nurse, supporting to collate historical information from relatives/carers of the person living with dementia

  • To work with care homes of Knowsley, identifying service users who appear to be living with dementia but with no formal diagnosis
  • Work with care homes to implement appropriate and personalised, holistic care plans
  • Support, educate and share information with families of the person living with dementia and care home staff

CLINICAL:

Participate in regular clinical supervision as per Trust policy.

Participate in practice/clinical supervision to monitor standards of service user care and documentation.

Assist with the appropriate screening of referrals and provide information for referrers, service uses and carers

Develop and maintain effective working partnerships with services in other statutory and non-statutory organisations

Assist in the development and implementation of integrated care pathways, including appropriate service user assessments/referrals and models of care.

Ensure the clinical environment is conducive to promote service users health, safety and well-being, thereby preserving and respecting each service user's personal dignity, privacy, religious and cultural
beliefs.

Ensure record keeping is in accordance with Professional and Trust standards.

Work in a professional and collaborative manner with the Multi-disciplinary team.

Participate and promote positive intra-agency working.


Responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care without supervision and act as Named Nurse as required.

Participate and assist in setting standards of service user care based on sound research findings.

Ensure that Trust policies and procedures are adhered to.

Assist in the development of local protocols to ensure a high standard of service delivery is achieved and maintained.


Support the process of staff participation in the formulation of new ideas/suggestions to improve service user care and service delivery.

Provide statistical information as required.

Ensure effective processes of communication are established, maintained and regularly evaluated within designated team.

Ensure that the nursing practice within the clinical area adheres to the Trust's policies and procedures.

Participate in managerial supervision.

Qualifications:

Essential criteria

  • Up to date Professional Registration (to be maintained), on the appropriate part of the NMC register or equivalent HPC register and hold a current NMC/HPC registration
