I am an elderly lady with Dementia, living at home in Peebles. I am becoming increasingly

frail and as such require more assistance with living at home as safely and as independently

as possible.

Main tasks and responsibilities

Morning (30 minutes on wash days and 1 hour on shower days)

Please come in to assist me with getting up from my bed and supporting me with a

safe transfer through to the bathroom.

Please assist me with showering/washing supervising at all times.

necessary creams, please then assist me with dressing.

Please then ensure a safe transfer through to the living area and assist me with

preparing my breakfast and prompting my medication.

Finally, please orientate me to the day of the week and current time and remind me

of any planned activities for the day.

Lunch (30 minutes)

Please come in to assist me with preparing my lunch and a hot drink of choice.
Please prompt me to take my medication.
Finally please orientate me to the day of the week and current time.

Tea (30 minutes)

Please come in to assist me with preparing my evening meal and a hot drink of

choice.

Please prompt me to take my medication.
Please ensure adequate fluid intake.

Bedtime (30 minutes)

Please come in and assit me through to bed.
Support me with getting into my nightwear if I have not already done so.
Finally, please ensure I have taken on adequate fluids and also have some to hand

for overnight.
