Description
The PBP is developing Integrated Neighbourhood Teams (INTs) as a mechanism for delivery which fully embrace joint working across our provider partners (inclusive of VCS as well as statutory bodies) and our communities and residents.
The capacity created via the Health Inequalities Investment Fund (HIIF) (Clinical and non-clinical leadership of the INTs) will facilitate the development of this more proactive approach to tackling Health Inequalities within each neighbourhood and the interventions will help to achieve better outcomes.
The post holder will be a key member of the wider Mid Notts Place Based Partnership (PBP) and will provide non-clinical coordination of the Mid Notts Integrated Neighbourhood Teams (INT) to which they have been assigned.
The post holder will work closely with the HIIF Clinical Lead, the Programme Manager, INT Clinical Lead for the INT to which they have been assigned and with the INT Clinical Leads for each of the PBP Themes.
The Mid Notts PBP Themes are Best Start Plus, Living Well, Ageing Well, Health Inequalities.The post holder will also work closely with other INT Clinical Leads, INT Coordinators and Place (ICB Locality) Team across Mid Notts to ensure the smooth running of the INTs and delivery of the agreed Outcomes.
ROLE OBJECTIVES:
PBPs are collaborative arrangements formed by organisations responsible for arranging and delivering health and social care services in a locality or community.
They are the foundations of the ICS as they build on existing local arrangements and relationships.
The post holder working closely with the GPs, Community Services, PCN Team, Local District Councils, Social Care, Voluntary sector organisations and will: Provide support for GP Practices, patients, carers and professionals to improve patient care access and navigate resources and services which will help those patient groups prioritised by the MN PBP and Clinical leads.
Ensure works to deadlines and prioritises workload, working towards KPIs and targets set for the service.
Main Duties and Responsibilities Improving Patient Care I Identify evidence based opportunities to address health inequalities and improve outcomes for vulnerable communities and be able to present those opportunities to various audiences.
Identify and agree key priorities for the local area with PBP Partners. Contribute to the health improvement and reduction of inequalities.
To act as a champion for INT, for patients, residents and communities and their interests and involve the public and patients in the design of the service model.
Ensure that the reduction of health inequalities is embedded into practices across the INT based on local population need, evidence of best practice and effectiveness.
Develop Collaborative working and Communications with MN PBP Collaborate with partners, community sector organisations and community representatives to co-produce innovative development and re-design of services.
Organise meetings as requested.To include (not limited to) sourcing venue / setting up virtual meetings, finalise delegate list, invitations, track responses and liaise with organiser, invite speakers, organise refreshments and remain within budget (where applicable).
To act as meeting secretariat in the preparation of agendas and circulation of relevant papers/minutes to participants.Attend to minute the meetings and any other meetings as required and ensure production of good quality minutes and dissemination of minutes within agreed timescales.
Be sensitive to the local political landscape and sensitively work across organisations, often against conflicting priorities, to bring consensus enabling effective reduction of inequalities.
Work collaboratively with the INT Clinical Lead(s), local health care providers and wider system partners to support development and design of INT, modelling strong collaborative behaviours that focus on improving patient outcomes and encouraging engagement.
