Haringey Multi Agency Care Coordination Mental Health Professional
An exciting opportunity has arisen for full time Band 7 in the Haringey Multi Agency Care and Coordination Team. You will be the lead mental health specialist within the team.

You will join a multidisciplinary team of professionals in triaging, allocating assessments and providing high quality care to the residents of Haringey who have complex health conditions to receive timely care which will reduce and prevent unnecessary hospital admissions in the frailty.

The successful applicant will be based at the Lordship Lane Primary Care Centre N17 6AA which is a busy multicultural area of London.

You will have relevant professional qualification in Nursing, with 4 years post qualification experience.

Previous work at band 6 within the care of the frailty will be essential as well as having a good multidisciplinary working relationship and experience in implementing appropriate care pathways.

Excellent, communication skills are essential and should be evident in presentations.

You will have the ability to work under your own initiative as well as in collaboration with colleagues to meet deadlines.

You will also have the ability to update and/or implement new systems in line with Trust and service requirements.
