An opportunity has arisen to work in an exciting new integrated model for the delivery of patient care in Dorset County Hospital's new Acute Hospital at Home service.

This service proactively manages and monitors a defined caseload of patients who have complex care needs in the short term, on a multi-disciplinary basis in their own home.

We are looking for a dynamic data & admin assistant, to work in our multi-disciplinary team, ensuring a patient focused and individualised approach.

Ideally you will haveall skills as outlined in job descriptionand have experience of effective multi-disciplinary team working in its widest sense.

To support the AHAH Team in all aspects of their role.

Maintain accurate and up to date computerised and manual records for patients and staff, including processing timesheets, data dashboards, project information and entering target data.

Ensure daily data is collected, collated, analysed and distributed in accordance with the AHAH Service requirements.

Prepare data for analysis by external bodies eg:
ICS.

Provide general administrative and data entry support, maintaining confidentiality at all times
