The Division of Integrated Care within Stockport Foundation Trust is seeking an Advanced Clinical Practitioner (ACP)- (Nursing /AHP/Therapist) to work within the urgent Crisis Response Team.
We also aid early supported discharges and admission avoidance with the innovative concept of a virtual ward to allow patients to receive the care and monitoring they need at home safely and conveniently.

The post holder will be practising autonomously as an advanced practitioner within the urgent Crisis Response Team providing patient-centred clinical care.
The post holder will support new ways of working that focuses on efficient and patient focused service, and will ensure the safe treatment, referral and discharge of patients with undifferentiated and undiagnosed presentations in their area.

The post holder will deliver an 80% clinical component to their role and 20% related to appraisal, clinical audit, teaching, self-development and research.
Undertake independent comprehensive assessment and management of patients to incorporate physiological and pharmacological considerations, making multifaceted complex clinical decisions regarding patient management and clinical outcomes.
The post holder will be responsible for providing advanced clinical care and expertise in care delivery including judgement, discretion and decision making to patients as per individual scope of practice and through the advanced clinical competency framework provided by the MSc programme.
The post holder will provide a seamless, high quality service from referral through to assessment, diagnosis, treatment and review, referring to other specialists as required.
