The community hub team offers a range of specialist health interventions to people with a learning disability within their place of normal residence and / or respite placements.

The interventions offered are conducted within the agreed pathways offered by the Learning Disability Service:

Physical Health

Mental Health

Behaviours of Concern

The successful applicant will be responsible for supporting the hub of multidisciplinary practitioners including consultant clinics.

We are looking for a motivated person to join our team. You must be organised, have effective management skills. You will also need to have good communication skills and the willingness to develop.

This is a developing team and we will be seeking new ways of working so we need someone who can contribute to the ongoing development of the team and responds positively to change.


Applicants must be able to work a regular shift pattern of 9am - 5pm Monday, Thursday and Friday but must be flexible as per service need.

The applicant should ideally have experience of supporting people with learning disabilities however training could be provided.
