Community-based working. Will need driving licence & access to a car.

This is an exciting opportunity to join one of our excellent district nursing teams.


We are looking for a group of highly motivated applicants who are interested in meeting the challenge of working on the community with patients in the home setting.

You will be working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients to ensure their independence and well being.

The post holder will carry out a variety of nursing duties as part of the patients holistic care management through the assessment and evaluation of health needs particularly in the areas of palliative care, wound management, diabetes and other long term conditions.


The post holder will be expected to work multi professionally, support other team member's roles and contribute to the future development of the service.


As a senior nurse you will be able to demonstrate good leadership skills, the ability to manage change in clinical practice and processes.

You will also be able to demonstrate the ability to independently make sound clinical and leadership judgements and regularly supervise the workload of junior colleagues.

We are looking for people who are innovative, forward thinking, flexible and have strong leadership qualities.
