About the Role:


As a Mental Health Practitioner in our Duty Support and Care Coordination Team, youll play a vital role in providing comprehensive care coordination to a diverse caseload of patients.

Youll be responsible for ensuring seamless and effective care plans, facilitating access to appropriate resources, and collaborating with a multidisciplinary team to ensure holistic patient care.


Key Responsibilities:

Care coordination for a caseload of patients
Collaborate with healthcare professionals to develop and implement care plans
Provide crisis intervention and support
Contribute to duty rota cover
Maintain accurate and up-to-date patient records
Deliver compassionate and person-centered care


What Were Looking For:

Band 6 Qualified Mental Health Practitioner
Experience in care coordination and patient support
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Compassionate and empathetic approach to patient care
