Specialist Medicine- Ward 30 LRI
We are looking for people who will be able to work on their initiative as well as part of a team.
The Wards can have a high turnover of patients; as well as complex and complicated discharge for patients on End of Life pathways.
Early identification of potential needs and additional support is very important in achieving a short length of stay and reduces risk of potential readmission.
This role with include full time hours/ shift times to be discussed but may include weekends and bank holidays.
To work closely with the ESM Discharge Team to promote a discharge ethos amongst the wider MDT, supporting the UHL beds and base wards programme which ensures that strategic and operational plans meet the needs of the specialism.
- To focus directly on the needs of the patients requiring support and facilitation to be discharged from the acute hospital
- To develop close working relationships with your ward teams, providing clinical and administrative support to the multiprofessional team, having delegated responsibility under the direction of an appropriate registered healthcare professional
- Act as a clinical expert and professional resource for patients, their carers and the multiprofessional team in relation to the discharge process.
- To drive a team culture alongside colleagues working together.
- Act as an advisor regarding discharge to community settings and various other discharge destinations
