Introduction:


This role is in partnership with The Wallich, please note the successful applicant will be employed by Kaleidoscope.


Kaleidoscope is a pioneering substance misuse treatment provider, offering evidence-based treatment and holistic support to individuals who access its' services.

Staff teams work collaboratively with partner agencies including The Wallich, G4S, Local Health Board, South Wales Police, HMPPS (Her Majesties Prison and Probation Service), PHW (Public Health Wales), and other third sector support agencies.


Clinical services include substitute prescribing for opiate dependence, harm reduction initiatives including the provision of take home Naloxone; family planning and sexual health screening; Blood Borne Virus testing and referral on to specialist support.

Service users will be offered a full range of support to assist them in overcoming their substance use.

Treatment will be individualised via personal recovery care planning and regular clinical assessment and reviews, and will seek to support service users to address their substance use as well as to raise motivation to address wider social and health issues.


The Harm Reduction Role will deliver services in line with Welsh Government Treatment Frameworks, NICE guidance (National Institute for Health and Clinical Excellence), and other relevant professional standards and legislation.

Kaleidoscope staff are expected to follow Operational Policies and Standard Operating Procedures (SOPs), ensuring that the care and treatment provided is safe, undertaken to a high standard and informed by up-to-date, evidence-based practice.


Project Description:


Housing First is based around the ethos that housing is a human right and differs from traditional supported housing and homelessness provision that places a number of conditions on people to prove that they are ready for independent housing.


Purpose of Role:

Main Duties, Responsibilities and Person Profile:
a) To reduce drug and alcohol related harm to the HF cohort, including reducing harms associated with use of substances across the life course - whilst improving wellbeing by tapping into, and facilitating access into community and specialist resources available

b)To provide the following Harm Reduction interventions:

  • Safer injecting advice
  • Polydrug use advice
  • NPS HR (including spice)
  • Alcohol Screening and withdrawal advice
  • Mental Health Assessment and referral on
  • Alcohol Related Brian Damage mini assessment and referral on
  • Alcohol brief interventions
  • Nutrition advice
  • Stimulant advice (Crack cocaine)
  • Opiate Substitution Therapy advice
  • Overdose Prevention / Awareness
  • Naloxone training and administration
  • Hospital Liaison
  • Drug Screen Urine Analysis testing and compliance monitoring
  • Alcohol Breath Screening
c) To promote healthier lifestyles and enable service users to improve their own health and
wellbeing.

d) Ensure BBV screening and Hepatitis B vaccinations are comprehensively delivered

e)To promote and facilitate individuals into both abstinence based and medically assisted
forms of recovery

f) In enhancing the Needle exchange offer - internally and within pharmacies

g)To deliver naloxone training and distribution.

h) Increase partnership working and improve the knowledge of relevant staff in relation to
substance use and associated behaviours

i) To deliver harm reduction education, awareness, and prevention training to both service
users and Housing First workers.

j) To initiate smoking cessation advice, linkage, and other health related support

k) Undertake comprehensive clinical assessments of health needs.

l) Administer methadone to clients accessing prescribing services, adhering strictly to dispensing
procedures (SOPS).

m) Supervise medication consumption by individuals accessing our prescribing services.

n) Ensure case notes and other statutory records are up to date and kept in accordance with
Kaleidoscope policies and procedures/regional information sharing protocols.

o) Provide support and cover colleagues delivering services when required.

p) Participate in clinical audits, service reviews and the evaluation of service user satisfaction,
implementing resulting recommendations, to improve and develop the service.

q) Actively encourage service user involvement and feedback and ensure views are considered
in informing the development of the service.

r) Collate data and contribute to service reports.

s) Work within agreed policies, procedures, and protocols to safeguard children and
vulnerable adults.

t) To act as the lead for ensuring testing and vaccinations across Housing First are being
undertaken, recorded accurately, monitored, and cleansed alongside compiling data
required by stakeholders and Housing First Project Manager.
