Senior Care Home Practitioner Band 7


This post holder carries responsibility for the assessment and identification of residents living in a Care Home and will support the care home in identifying patients for the Multi-Disciplinary Team Meetings held weekly in each Care Home (MDT).

The post holder will assist the care home staff in providing a comprehensive, advanced assessment, practical problem solving and case management approach to the management of residents with complex long-term conditions.

Whilst this is a dedicated care home role, the post holder will also be a member of the Integrated
Community Team.

They will participate in providing a more coordinated practical problem-solving approach with primary, secondary, social and the voluntary sector partners to improve patient experience, satisfaction and outcomes as part of a joined up approach in conjunction with the care home and PCN lead.

Use advanced skills and expert knowledge to support the Care Home in
identifying residents who will be reviewed at the Care Home Multidisciplinary Team Meeting (MDT)

Contribute to the decision making within the Virtual MDT including whether the resident would need a physical visit.
Be able to demonstrate the principles and practical aspects of shared decision making with patients and families as part of facilitating a self-management approach
Support the PCNs in developing a Personal Care Support Plan (PCSP) for each person living in a care home. This should include the resident /family/carer and taking into account mental capacity. In conjunction with the care home follow the standard model of personalized care set out
in the Universal Personalized Care principles.
Ensure that the PCSP includes the resident preferred place of death, Do Not Attempt Resuscitation (DNAR) and Advanced Care Plan (ACP)
Work with the care home in identifying the residents hydration and nutritional needs
