Seeking two autonomous, energetic and insightful system thinkers to support the implementation of the NHSE Patient Safety Incident Response Framework within the division of surgery at UHSussex.


This innovative role will focus on the investigation of patient safety incidents, providing a systems review and supporting the teams with the development of shared learning and improvement priorities.


The post holders will ensure the quality governance and compliance of patient safety investigation, reviewing the quality assurance of incident investigation reports so that they are of the highest standard prior to sharing with patients, families, staff and external stakeholders.

This role will also lead on timely reporting and submission, and compliance with Regulation 20 (Duty of Candour - DoC).

  • Leading by personal example, to establish a high profile for patient safety within the Trust and working with others to share good practice. Actively contribute to help communicate lessons learnt and celebrate success.
  • Provide the quality assurance methodology for the review of all PSII reports ensuring alignment and compliance to the NHSE Patient Safety Incident Response Framework (PSIRF).
  • Provide specialist support and advice with regard to investigation methodology and investigation report writing.
  • Undertake comprehensive patient safety incident investigations, after action reviews, hot debriefs and thematic reviews (based on a monthly trajectory set by Director of Patient Safety).
  • Working in collaboration with patients, families, carers and staff, produce high quality (based on national HSIB system based best practice) incident investigation reports.
  • Demonstrate good understanding and background knowledge of national systemsbased models such a SEIPS and Safety I & Safety II methodology.
  • Work alongside the divisions as part of the Trust Clinical Operating Model (COM) as an initial point of contact to advise and assist with incident management, and Duty of Candour.
