The team of ACP's will be the clinical leaders for the virtual wards, overseeing patient admissions, managing the nursing team to ensure all patients receive the care required.
The ACP's will be the liaison point with the consultants and acute clinical teams for patient escalation and progression.Between 8pm and 8am the ACP will be the point of contact for patients on the virtual ward and will use the agreed SOP's to escalate care to the relevant acute team when needed.
This is a training post to become an Advanced Care Practitioner (ACP).The successful applicant will work as part of the ACP team during their training period with the support and oversight of consultants and experienced ACP's.
