Locum Band 6 Community Rehab Physiotherapist - Berkshire

We have a fantastic opportunity for a locum Physiotherapist to join a leading Community hospital, based in Berkshire. They are seeking a qualified Physiotherapist with previous Community experience.

Enhanced DBS
Desirable postural and contracture management.
Community Rehab physiotherapy exercise programmes, strength, balance, transfers, mobility, intensive input for ICR patients post hospital discharge.
Identify patients who meet ICR criteria and coordinate ICR discharge once patients meet criteria
Integrated working between Acute / Community services / Social care/ voluntary sector
To holistically support the patient throughout the rehabilitation process and to have a patient centred approach
To ensure that patients have a clear rehabilitation plan identifying their needs and ensure appropriate actions are taken prior to discharge from Acute hospital
To undertake risk assessments and implement risk reduction measures in line with current policies and guidelines
To assist the patient in identifying their goals and to recognise when these have been achieved.
To liaise withIN MDT
To work as part of PACE Team (Post-Acute Care Enablement) and supporting team as required for smooth running of service.

Position Requirements

HCPC Registered
3 month (minimum) locum assignment to start as soon as possible
Full time hours (Monday
Friday)
Minimum of two years postgraduate experience
Previous clinical experience performing a similar role within a Community setting
