An opportunity for 1.5 WTE clinical call handlers has arisen to join the Digital Care Hub team to support the delivery of the new Virtual Ward service across Bradford Districts and Craven.

The Digital Care Hub provides video and telephone support to approximately 25,000 patients living in their own homes and in nursing and residential care.


The Hub is a 24/7 service, with the aim of supporting patients in their own place of residence where it is clinically safe to do so, reducing the number of unnecessary attendances at A&E and unplanned admissions.


The service is rapidly expanding and as such we need additional staff to offer greater support to the new innovations in our work.


Applicants will need to have an innovative and flexible approach to work and be receptive to the changes that occur as part of this evolving service.


We are looking for people who are interested in new ways of working and are keen to be part of both the new Virtual Ward and wider Digital Care Hub services.


Under the guidance and supervision of the ADCH Clinical Team, the post holder will work as a member of the Hub team to undertake a range of duties which include: taking calls from patients in their own homes on the Virtual Ward and/or MyCare24 service, registering residents from care homes and patients to all DCH services on to SystmOne, actioning tasks allocated by clinical team, maintaining own competence in documentation, education support and direct care activities to support patients, carers and family members.

The post holder will work to the Level 3 standards and complete the Trust competency booklet.

We are a national centre for telemedicine and introduced telehealth to the UK offender healthcare sector and also provide the service to patients in their care homes.
