Description

An exciting development opportunity has arisen to join our outstanding Trust as a smokefree advisor for a period of six months(potentially could be extended) covering maternity leave.

Are you a pharmacy technician or experienced health care assistant or any other health care professional with the interest in helping people to stop smoking? You will be part of a dynamic service delivering intense smoking cessation support to smokers admitted to hospital, outpatients and community services.

The successful applicants will need excellent communication, motivational and organisational skills and focus will be to promote our smoke free policy across our inpatient services and community services.

The ambition of LHCH is to support the NHS long-term plan to improve population health and this service will focus on long term care for people with heart and chest problems.

The full time Smokefree Advisors will have their own delegated caseloads, working under agreed clinical pathways and within defined competence.

Working under unsupervised guidance, they will be expected to motivate and engage sick smokers, provide individualised patient management plans whilst in hospital, facilitate follow-up support post-discharge and ensure effective handover of care to established community services.

Also, be part of community commissioned stop smoking services and actively provide smoking cessation with all community patients.

Interviews will be held on 31st January 2023.
