Description

Lead the operational management and the service delivery within the Discharge Team and provide support to the Bed Management Lead where indicated.

Provide leadership to the teams and ensure that managerial supervision, the appraisal process, and clinical supervision are being delivered regularly and effectively in line with Trust policy and procedure.

Establish clear governance procedures within the Teams to ensure good communication, learning and best practice is established.

Ensure tasks and responsibilities are effectively, fairly and evenly distributed within the teams so that effective and responsive care is provided for service users and carers Contribute to Trust wide or external projects and initiatives as required and in agreement with the Operational Flow and Capacity Lead.

Use data and information to develop an in depth understanding of resources, costs and activity and use this to improve performance of each team
