The Wokingham Community Older Adults Mental Health Team is looking for TWO enthusiastic, caring and highly motivated Senior Mental Health Practitioners ( Social Worker, Mental Health Nurse or Occupational Therapist ) to join our friendly and innovative Home Treatment Team (HTT).
Your base would be Wokingham Community Hospital where we have access to free parking facilities, staff room and a friendly and welcoming team.
The post holder will be well supported and guided by our Clinical Lead and HTT Lead as necessary and have day to day responsibility for their own allocated caseload and HTT caseload.
They will act as a keyworker assessing the patient's needs and providing high quality mental health interventions appropriate to the individual, including care management.

The "must haves" for you to be considered for this role:
- Have the relevant professional qualification ( Social Worker, Mental Health Nurse, Occupational Therapist ) and clinical mental health experience
- A passion and understanding for working with Older People
- Good understanding of the relevant legislation surrounding the Mental Capacity Act / Mental Health Act
- The ability to assess risk in both routine and urgent situations
- Be a car driver with the ability and readiness to travel
