We are looking for an enthusiastic, committed and hardworking person to join Failsafe officer

The role of the Failsafe Officer is essential to safeguard Diabetic Eye Screening
Programme (DESP) patients as they move through the complex diabetic eye screening
programme care pathway, escalating concerns or omissions to local DESP managers in
accordance with policies and procedures.

The post holder is directly responsible for delivering high quality data from audits, list
validations and liaising with multiple stakeholders. Following policies and procedures and
regularly audit for compliance.

  • Deliver high quality data from carrying out regular audits. Assisting in collection of
data used within the department to indicate performance against local commissioning
targets and KPI's.

  • Ensure communication channels between DESP and Hospital Eye Services and Key
Stakeholders remain clear, to ensure that all patients are in the correct pathway,
supported by systematic auditing and tracking.

  • Accountable for liaising with Hospital Eye Services to ensure patients referred
receive an initial appointment within NDESP criteria and track their status ensuring
they return to the care of the DESP on discharge from the Hospital Eye Services.

  • Responsible for monitoring waiting times for the Hospital Eye Service assessment
and treatment pathway, escalating and potential breaches to the Failsafe Team
Leader.

  • Regularly review patient cohort to ensure all patients are accounted for and regularly

audited (including but not limited to: Opted out, medically unfit, not diabetic,
pregnant). This will include liaising with stakeholders involved with patient care, for

example:
GP's, practice nurses and administrative teams, diabetologists,
obstetricians and paediatricians.

  • Support the administration team with queries, as well as telephone support for
patients who require information regarding their appointments. Booking,
rearrangement and cancellation of appointments in accordance with programme
requirements.

  • Monitor various Hospital Eye software's to ensure that all referred patients are seen
within the NDESP recommended timescales. Collecting data on patient appointments
to update internal software's with accurate information. Recalling patients to DESP
where appropriate.

  • Maintain validating of the software and cleansing the single collated list. Requiring an
excellent level of attention to detail.

  • Analyse and validate regular spreadsheets sent to DESP via Hospital Eye Services
for upload to internal software.
