We are seeking an enthusiastic Band 3 Patient Flow Coordinator who is passionate about high quality patient care to support effective patient flow within the Trust.

This is an exciting time to be joining us as our team continues to expand.


Whilst you will be operating under the patient flow team this role is ward based, and will be based within the Rotherham care group.


If you are highly organised and can effectively communicate with staff at all level and have good patient care at the heart of everything you do then we are looking for you to take on this exciting and new role.

Experience of working in mental health and within inpatient settings is advantageous but not essential if you have the transferrable skills we are seeking.

To work as a member of the Patient Flow Team and provide support to the Care Group Multi-Disciplinary Team (MDT) to ensure patients' timely discharge from hospital, including participating in developing discharge plans and identifying barriers.

This requires the post holder to work within a specific locality developing an understanding of issues that may prove to be a barrier to discharge.


Support the ward team in the completion of tasks necessary to achieve an individual's discharge, including making and following up onward referrals and tracking progress.

Work creatively to engage patients and assist in aspects of discharging planning.

This will include progressing any barriers that could delay discharge by developing strong working relationships with Care Co-ordinator's/Led Professionals in the wider MDT, partner organisations and outside agencies.


Work in partnership with relevant organisations like housing providers, social care providers and voluntary / community organisations to support patients to access appropriate services that will continue to improve their mental health and recovery on discharge, promote independence and support a safe discharge from hospital.
