Are you a rehabilitation support worker (RSW)/ therapy assistant with interest and experience of developing others? Are you enthusiastic about the opportunity to be part of the Rehab and Reablement team that regularly celebrates success and positive outcomes.

If so, we have an exciting opportunity for a Band month fixed-term Rehabilitation Support Team Lead role within our multidisciplinary team (MDT).


Supported by the clinical leadership team, you will have a varied workload overseeing the day-to-day operations of the RSW workforce whilst working with patients who have varying complex needs both independently and through the clinical supervisions of others.


Embracing the power of technology in enhancing patient care, we use EPIC, Rover and Dragon to enable us to take "on-the-go" notes.

This reduces the level of paperwork that builds up and managing workload evenly throughout the day, allowing us to focus on the most important aspect of our role, our patients.


We are an integrated health and social care MDT, working with adults over the age of 18 who have experienced a recent change in their function secondary to physical and/or cognitive changes.

We provide up to 6 weeks of tailored rehab that optimises their engagement and independence of their activities of daily living.

Intermediate Care Lambeth provides integrated health and social care at home to adults aged 19 years and older who live in the borough of Lambeth.

The people that we support have a range of diagnoses and clinical presentations.

Facilitate the safe discharge of people from healthcare in hospital to healthcare at home.
Provide intensive, short term multi-disciplinary rehabilitation and care support to promote functional independence.

This role is to support the Deputy Clinical Lead in the delivery of departmental duties within the Senior Rehabilitation Support Worker (SRSW) and Rehabilitation Support Worker (RSW) teams within the Intermediate Care Lambeth service.

This will involve delivering against targets whilst ensuring the provision of a high quality multidisciplinary secretarial service to patients and clinical teams within the service.


This post would assume the oversight of the SRSW and RSW teams supporting their line managers in co-ordinating leave, the development and implementation of development opportunities, rota and HR management.
