The
Admission Prevention Community Team is a developing team and we are looking for enthusiastic and creative people who enjoy a challenge to come and join us.

What does it involve?


You will be going out to meet people in the community who are finding it difficult to manage and who may well be on the cusp of a hospital admission or have recently been discharged from hospital.

You will recognize that even in the most difficult of situations, people are often relying on a range of strengths and skills to cope and survive.

You will work alongside them, proactively helping them to identify the things that will help them to stay well in the community.

You will use your ability to connect with people and develop their trust to help them to quickly navigate the system and overcome barriers.

We all have experience of one sort or another.

You may have worked in other services that are similar, or you may have worked in a completely different roles, but you get who we are, are naturally strengths focussed and person-centred in your approach, and you know that you could really make a difference.

Tell me a bit more about the job?


The Admission Prevention Community Team operates in Norfolk 7 days a week, including bank holidays, between the hours of 09:30-20:00.

There are both full time and part time roles available and flexible working patterns will be considered, working hours and patterns can be discussed at interview.

A typical day would consist of visiting vulnerable adults with mental health needs in the community to provide a variety of support including, emotional support, social inclusion activities, signposting and supporting to engage in services in the community.

You will be working within a friendly and knowledgeable team and will receive a comprehensive induction when you join us as well as training through our annual cycle.


Please note - you will need to have access to a car for business use due to the outreach nature of the service.
