Description

To work in conjunction and communicate with all members of the Hospice at Home team, and departmental manager to ensure that they are aware of any new referrals and of any plans for discharges.

Distribution of daily work sheets to the day and night shifts.

Planning and organisation of tasks, some of which may be ongoing including ordering of equipment to patients needs under nurse guidance.

To ensure that communication with families is maintained and that they are given the opportunity to discuss any fears they may have.

This may include the organising of follow up bereavement support and guidance. To facilitate and support staff to ensure smooth discharge planning. To create and maintain good working relationships with external agencies.

To take responsibility for ensuring that all documentation relating to admissions and discharges is up to date and relevant, including Continuing Healthcare (CHC) assessments and similar.

To process and document access to fast-track funding (training will be given). Support the FHCT Office Coordinator and deputise during periods of absence.
