12-month Secondment Vacancy


The Patient Flow Coordinator will lead on the tracking of pathways of patients' who are suspected of having or have confirmed cancer.

By working closely with the (multi-disciplinary team) MDT Clinical Lead, Pathway Manager and Service Managers and providing high standard of administrative support, the post holder will ensure cancer patients receive treatment as soon as possible and ensure national targets are met.


The post holder is responsible for providing a high standard of professional and patient focused service to a range of Trust-wide specialties and will support MDT functions on a day-to day operational basis.

This position is an opportunity to directly improve the care of cancer patients.

By monitoring and reviewing patient care pathways and/or treatment progress, the successful applicant will have the opportunity to work closely with administrative and clinical colleagues to identify and suggest improvements to ensure cancer patients receive treatment as soon as possible.


Strong communication skills and the ability to work with others within different roles within the Trust are key to this position, as is strong administrative experience and the drive to improve patient care.

Operational

Proactively manage case load of patients to ensure treatment of confirmed or suspected cancer as soon as possible


By:


Monitoring and reviewing patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies.

Identifying from Tracking List next steps in patient care pathway in line with clinical need (peer review).

Receiving patient referrals and administer in line with timings and requirements as set out in Trust and National policies.


Updating on all enquiries regarding patients' care and/or treatment on a cancer pathway from external Trusts and General Practitioners in a timely manner.

Developing an understanding of milestones of allocated pathway for patients within designated tumour group.


To participate in national cancer audits and collect data as required in MDT meetings, allowing the Trust to identify and improve treatment in patient cohorts.

Identify and suggest improvements that can be made to the patient care pathway and/or the patient pathway tracking process.

Communication

To attend MDT meetings and complete actions regarding the care of patients within the designated pathway.

Develop working relationships with clinicians and managers across the directorate.


Monitoring patients on their pathway and proactively finding resolutions to improve the speed of treatment by working directly with clinicians and managers.

Action day-to-day issues, ensuring a resolution. Escalating serious issues to the Pathway Manager.


Responsible for updating patient information on the Cancer PTL system (Somerset Cancer Registry) in line with local policy, to ensure efficient pathway progress.

Ensuring other users update Somerset Cancer Registry in the required timeframe within the designated pathway.

Escalate any issues and breaches of the Cancer Waiting Times standards to the relevant management in line with agreed escalation procedures.

Policy development

Review and support development of Standard Operating Procedures, to ensure these are adhered to.

Review and support development of internal targets, to ensure these are adhered to.

Staff Management

Provide day to day supervision and oversight of duties for the MDT Administrative Assistant.

Ensure the Administrative Assistant completes preparation for each MDT.
