We are a wholly independent local charity affiliated to the Age UK movement in the UK through a Brand Partnership agreement.

We are also keen to focus on the development of new, innovative services which will support us in achieving our mission to enhance the health and well-being of older people in Herefordshire and Worcestershire

Age UK Herefordshire and Worcestershire is currently undergoing an extensive and exciting programme of modernisation and forward thinking, encompassing a review of its existing service, financial and workforce model, developing a rejuvenated culture aligned to our core vision and values to improve our overall people experience.

ABOUT THE ROLE

To enable us to continue to meet our customer's needs, we are seeking a motivated and Health and Social Care professional to work collaboratively with colleagues to develop and coordinate the delivery of services to the residents of Herefordshire.

You will oversee and provide day to day overall guidance and management of our Drybridge Meeting Centre to develop an effective Meeting Centre based wellness programme.

You will also work as part of the team to coordinate delivery of a Hospital Discharge Service, you will liaise with health professionals, the patient, the Home from Hospital Team to ensure timely and safe discharge from hospital.
