An exciting opportunity has arisen to join an established but evolving mental health Care Homes Service.

The LYPFT Care Homes Team is seeking a Band 5 Occupational Therapist or Registered Nurse (MH) to join a small, friendly service that directly supports older people with complex dementia /mental health needs residing in a Leeds care home.

You will have post registration experience and be able to demonstrate an in depth understanding of the needs of Service Users with dementia and with severe, enduring and complex mental health needs.

We shall provide you with specialist training and development to ensure you are fully supported in this role.

Broadly, the model of service delivery includes: care coordination; timely assessment, role modelling appropriate behaviours / techniques, education for care staff tailored to the needs of the individual service user, fast access to mental health, psychology and pharmacological advice.
