Description

Responsible for engaging new individuals onto the service by working closely in partnership with treatment and recovery providers and with the recovery community.

Form strong professional relationships with the Emergency Department team and work closely with them to identify patients who may benefit from the Navigator service.

The Patient Coordinator will deliver, co-ordinate and contribute to a recovery programme for service users with alcohol misuse issues, including the delivery practical, bespoke interventions both face to face and with a network of partner agencies, ensuring that patients are offered the right interventions in the right place, at the right time.

They will support the patient through the journey from identification and referral to conclusion of their intervention, eliminating barriers to care.


The Patient Coordinator will work closely with Emergency Department Team and Alcohol Care Team, and partner agencies to ensure a patient focussed service, and will keep accurate patient and activity records to facilitate reporting back to Commissioners regarding performance and service success.
