Akton recruitment are currently working with a Local Authority in Islington who are looking to recruit a Qualified Occupational Therapist/Physio Therapist to join their team.


The Discharge Service involves the integration of the Hospital and Intermediate Care Social Work teams with the Discharge to Assess, and Admissions Avoidance Teams and Reablement to create a new service that will manage all incoming referrals for hospitaldischarge and admission avoidance.

The Discharge Service operates 7 days a week, 8am to 8pm. EDT provide support for all hospital discharges outside these hours.


Duties and responsibilities:

*To undertake a comprehensive advanced assessment of patients including those with diverse or complex presentations/multi-pathologies; use advanced clinical reasoning skills and manual assessment techniques eg auscultation/palpation to provide an accurate diagnosisof their condition.
*To be responsible for organising and planning own caseload to meet service and patient priorities. Readjusting plans as situations change/arise using appropriate clinical prioritisation skills.
*To provide recommendations for ongoing physiotherapy input for patients to physiotherapy departments within and outwith the acute setting eg other wards/hospital, community teams.
*To be professionally and legally accountable for all aspects of own work, including an identified caseload of patients.
*May be required to undertake visits to patient's home from hospital with other health/social staff with a view to discharge patients home.
*Work multi-professionally with consultants, nurses, GPs and other health care professionals and agencies. This may be one to one discussions, report writing and team meetings etc. This will include discussion of patient care, patient progress and involvementin discharge planning/onward referral.
*Evaluate patient progress, reassess and adapt treatment programmes if required.
