About the Role:
Sandhills PCN are recruiting - This is an exciting time to join an innovative and forward thinking Primary Care Network in North East Bedfordshire with a strong reputation for quality improvement and investing in people.
We are committed to improving the care provided to patients, reducing health inequalities and raising the quality and standards of GP practice.
We are looking for a dedicated individual to work with the Network Member Practices in order to deliver integrated care based around the needs of our local population.
Essential
- Experience of working within healthcare or the voluntary/community sector, supporting vulnerable groups to include delivering personcentred support in a nonjudgmental way and of completing holistic person centred care planning assessments.
- Excellent IT Skills including Microsoft Office and experience with GP Clinical System (Emis or equivalent)
- Strong listening and communication skills;
- Ability to work well in a team however also able to work independently without supervision
- Ability to review and improve working practices with shared experience across the PCN.
- You will be familiar with local resources and services and how to access them;
- You will be motivated by helping people, with care and empathy
Desirable
- Ideally you will have a Diploma Level 2 in Health and Social Care or equivalent qualification
About Us:
Sandhills PCN provides services to approximately 30000 patients across the towns of Biggleswade and Sandy and the surrounding villages. Sandhills PCN is headed by a proactive, supportive and committed leadership team lead by Clinical Director and GP Partners.
Our PCN includes the following practices:
Saffron Health Partnership and Sandy Health Centre.
We use SystmOne clinical system and consult with our patients using a range of methods from face to face, video, telephone and via video.
We provide an enhanced health in care homes service to the 5 care homes across our area.
