Key Tasks and Responsibilities of the Post Please note this is a band 2 job description and the role involves working toward the band 3 job description as attached.

1.To work under the direction of a registered member of nursing staff, but at times will not have direct supervision.

2.To carry out efficiently and effectively task and instructions assigned by the health professional. 3.To undertake only those tasks for which she / he has had instruction and is competent to fulfill.


  • To maintain confidentiality in respect of the patient / client at all times. 5.To be aware of Health and Safety at Work Act and the implications for the work environment. 6.To know how to report accidents and untoward incidents and to do so promptly. 7.To obtain knowledge of Organisational policies, procedures and practices and carry out duties in accordance with them.
To have an enhanced knowledge of policies pertinent to your area of work. 8.To be aware of legal issues and have an understanding of the Mental Health Act / Mental Capacity Act. 9.To support community activities either on a 1:1 basis or in small groups. 10.To be an active member of the provision of care to the patient / client.


  • Co-facilitate groups with health professionals. 12.To receive reports on patient care and be able to report back with observations to the nurse in charge within his / her competency. 13.To be willing to participate actively in appropriate in-service training programmes which the manager believes to be of benefit to the individual. To assume co-responsibility for maintaining skills and knowledge levels.
  • To assist the registered nurse to carry out, under direction, tasks which fulfil the patients / clients physical and psychological need, as identified in the Care Plan / Assessment. 15.Escort patients to other wards / departments or other areas designated by trained staff, which may include the community. 16.Contribute to the assessment, planning, implementing and evaluating care to patients. This to include participation in case review and assessment for admission / discharge process.
  • Make appropriate records in the multi-disciplinary patient file, which will then be counter signed by a registered nurse. 18.To assist the patients personal care and record patients observations in accordance with the Care Plan. 19.Record and manage patients property. 20.Undertake specific responsibilities designated by the Ward Manager.
  • To undertake mandatory and other training as directed by the Organisation. 22.To participate in continuing role / personal development using the appraisal system. 23.To participate in line management support and supervision. 24.To contribute to any audit process.
  • To behave in a manner which does not bring the Organisation into disrepute, whilst on or off duty. To comply with all conditions within contract. 26.To participate in the rotation programme and to work flexibly across the Mental Health Localities, according to service need. 27.To participate in physical intervention when applicable 28.To adhere to Livewell SW dress code 29.The post holder is required to comply with all infection control, policies and guidance, attend relevant updates and report issues of concern to their immediate line manager (if no action or explanation received, then it is the individuals responsibility to escalate to Director of Operations / Chief Executive office).
